Paxton Street Home Paxton Street Home is a licensed personal care home with a capacity of 85 residents. We serve adults of all ages who, for varying reasons, are unable to live in independent housing, but are able to self-direct many of their daily activities. Staff members provide support and encouragement to enable each resident to participate in room care, maintaining personal hygiene, and performing other daily life tasks. We provide assistance with shopping, attending physician appointments, and doing laundry while encouraging residents to live as independently as possible. Three meals a day are provided at scheduled times if the resident desires.
Our staff provides medication monitoring four times a day if needed for those who are on oral medications. While Paxton Street Home is unable to provide medical treatment and management, the staff is dedicated to advocating for the needs of the resident and communicating with physicians and other caregivers. Many of our staff members have extensive training and experience working with individuals who have chronic mental illness. Paxton Street Home provides a holistic daily activity program for those who choose to participate. Included on the monthly schedule are home-based activities such as crafts, Bible studies, exercise, and educational programs. A unique component of Paxton Street Home is that 8-10 of our staff members also live in the building, regularly interacting with residents outside of work hours and helping to provide a family-like home environment for all residents.
Admissions You can obtain the form by contacting the Director of Social Services or downloading them from the links above. Because Paxton Street Home is partially-funded through Dauphin County MH/MR, Paxton Street Home also accepts referrals from mental health case managers that have been approved by the Dauphin County Mental Health office. Once a Waiting List Form, Application or referral is received, the Director of Social Services will set up a tour and interview with the applicant and their family to determine if the applicant’s needs may be met by the home. Paxton Street Home accepts residents regardless of their ability to pay the cost of care, however applicants are asked to provide information on all income and resources to determine what their required monthly occupancy fee would be, based on out sliding-scale fee structure. If an applicant is accepted on the waiting list and an opening at the home occurs, the applicant is then required to obtain a Medical Evaluation documented on the state-required form and submit any other health records requested by the Director of Social Services before final approval for admission.
